COTW 02/21: A 77-year-old male with abdominal pain.
A 77-year-old male with a PMH of Cholangiocarcinoma with mets, recurrent ascites, multiple small bowel obstructions who presents to the ED with worsening abdominal pain for 2 days. Patient states he has a hernia that has progressively become worse in pain and now notice bruising around the site.
BP: 87/57 HR: 74 RR:16 SpO2: 100% RA
PE: Umbilical hernia noted with purple discoloration, no crepitus, tenderness noted to palpation.
